HIPAA Policy & Clinical Data Privacy Notice

Effective Date: February 18th, 2026

At Hawaii Facial Plastic Surgery, we are committed to maintaining the highest standards of privacy and data security for our patients. This notice outlines your rights and our responsibilities under the Health Insurance Portability and Accountability Act (HIPAA) regarding how your information is collected, stored, and utilized.

1. Website Interaction & Patient Inquiries

When you use our website to research aesthetic procedures or sign up for educational resources, we collect specific data to facilitate your inquiry. This includes:

  • Educational Leads: Information submitted via web forms (such as name, email, and phone number) to receive procedure guides, patient resources, or recovery tips.
  • Provider Consultation: If you request an appointment or information regarding specific surgical or non-surgical treatments, your contact information is used to facilitate a consultation with our licensed medical team.
  • Clinical Inquiries: If you express interest in specific treatments or clinical outcomes, the preliminary information you provide is treated as potentially sensitive data even before it enters your formal medical record.

2. Our Role as Your Care Provider

While we utilize advanced clinical technologies, Hawaii Facial Plastic Surgery is a direct treatment facility. When your health information enters our clinical systems, we act as a covered entity and data custodian. Our legal obligations include:

  • Maintaining the absolute confidentiality of your data within our clinical records.
  • Implementing strict administrative and technical safeguards to prevent unauthorized access.
  • Promptly notifying you if any data compromise occurs.

3. Utilization of Clinical Data

Your information is primarily used for your direct patient management. Additionally, we may use data for specific clinical and scientific functions:

  • Treatment Efficacy Tracking: We monitor functional and aesthetic outcomes to refine our surgical techniques and clinical protocols.
  • Safety Monitoring: We analyze clinical data to ensure all medical devices and biologics used in our practice meet the highest safety standards.
  • Scientific De-Identification: Before using any clinical findings for peer-reviewed publications or educational presentations, we remove all 18 HIPAA identifiers. This de-identified data is no longer considered Protected Health Information (PHI).

4. Digital Security Infrastructure

Our digital environment is designed to protect your privacy through rigorous standards:

  • End-to-End Encryption: All data transmitted through our website forms or patient portals is protected via industry-leading encryption.
  • Protocol-Based Access: Access to identifiable data is strictly restricted to authorized medical personnel and compliance officers under “Minimum Necessary” standards.
  • Data Sovereignty: Our clinical and research databases are logically isolated from our general business and marketing operations to ensure your health information remains private.

5. Your Rights Regarding Your Data

Regardless of whether you are a prospective lead or an active patient, you have the following rights:

  • Right to Withdraw: You may revoke your authorization for your data to be used for optional clinical research or educational registries at any time.
  • Right to Inspection: You may request a summary or copy of the personal health information we maintain in your medical files.
  • Right to Accounting: You may request a report of any instances where your information was shared for reasons other than standard treatment protocols or regulatory compliance.

6. No Marketing or Third-Party Sales

Hawaii Facial Plastic Surgery does not sell patient data. We will never share your PHI or website sign-up information with third-party marketers for their commercial gain. Your data is used exclusively for your care and the advancement of clinical excellence.
